Build a form
Make a form, like an application or feedback, add its questions, and choose who completes it and who reviews what's sent.
You need permission to build forms (forms.manage).
Forms are for things like applications, feedback and reports. People complete them at your community's web address, and reviewers accept or decline what they send.
Create a form
- Go to Forms and press Create form.
- Type the Title. The panel shows the form's address, made from the title.
- Choose where to Start from:
- A blank form.
- An application, with a few questions to start.
- A test, scored, with correct answers and a passing score. See Score a form or make a test.
- A moderator report. Accepting one opens a Moderation case about the person in its first SL avatar question, so that question stays required.
- Press Create form.
The form's page opens. Changes save as you make them.
Add questions
On the Questions tab, the Fields list shows each kind of field as it looks on the form. Click one to add it at the end, or drag it onto the form.
- Text and numbers: text, long text, number and link.
- Choices: one choice, several choices, a checkbox, a switch, and a sortable list people put in order.
- Scales: rating, slider and opinion scale.
- Dates and times: date, time, date and time, and weekly availability, all in SLT.
- Tables: a grid of statements with the same answers, and a table people add rows to.
- Second Life and Civitas: an SL avatar, an SL place, a role, a team or an SL group. Roleplay communities can also ask for a character.
- Files and signatures: picture uploads, and a signature people type or draw.
- Hidden value: saved from the form's link. See Share a form.
- Layout and display: section headings, columns, dividers, headings with text, and pictures.
Give each question its label, and Help (optional) to explain it. Check Required if it must be answered. Drag a question by its handle, or use the arrows, to move it. Anything but a section heading or a hidden value can go in a column.
Add checks and conditions
Under a question:
- Checks set what an answer must be, like the fewest and most characters, a number's lowest and highest, a date at least 18 years ago, or that an SL avatar is a member. Message when an answer fails its checks (optional) replaces what the check says.
- Show only if shows the question only when earlier answers match, or when the person holds a role or is on a team. Choose All of these or Any of these.
- Required only if makes it required only when the conditions hold.
- Skip ahead on a section jumps to a later section when the answers match.
Choose a layout
In Layout, choose:
- One page: every question on one page, with sections as headings. Check People can fold it away on a section to let people fold it.
- Tabs: each section is a tab.
- Steps: one section at a time, with Back, Next and a progress bar.
- One question at a time: good for short surveys.
Tabs and steps are made from sections, so add a section heading where each one starts. Review step before sending shows every answer before the form is sent. Save and finish later lets people save their answers for 30 days. Anonymous forms keep them only in the visitor's browser.
Choose who completes it and reviews it
On the Settings tab:
- In Who completes it, choose Who:
- Anyone signed in.
- Members: everyone in your SL groups. Members without a Civitas account sign in with a code from SL first.
- Certain roles or teams: check them. A team can be from a rank and up.
- Anyone, anonymously: nobody signs in, and Civitas keeps nothing about who sent it. Reviewers can't reply, and it can't ask for a signature or a character.
- Check Once per avatar to take one submission from each avatar.
- In Who reviews it, check roles to limit reviewing to people holding them. Leave them all unchecked for everyone who can review submissions. The owner always can.
A form that people have sent signed in can't become anonymous. Make a new one instead.
Open it
Turn on Taking submissions when it's ready. To open or close it at set times, choose Opens (optional) and Closes (optional), each with a time in SLT. The form's state shows at the top: Draft, Open, Opens later or Closed.
In About the form, you can also change the Title (the address stays the same), add a Description (optional), and write a Message after sending (optional). Add a message that depends on the answers shows a different message when the conditions you choose hold.
Change a form later
Changes apply at once. Once someone has sent the form, a change starts a new version, and what they sent keeps the questions they saw. Removing a question keeps the answers already sent.
Archive or delete a form
A form nobody has sent can be deleted with Delete. Once someone has sent it, turn off Taking submissions and press Archive instead. Its submissions stay, and Unarchive brings it back.
